How to Enter
Activate your MySmile Account
Visit www.deltadentalwa.com/mysmile and activate your account. Once you do, you’ll be automatically enrolled in a chance to win! All you need is your date of birth and an email address.
OR
Get Your Teeth Cleaned
If you already have an active MySmile account, simply visit your dentist for a teeth cleaning. We’ll know you went because we will send you an EOB (explanation of benefits) after your appointment.
How it Works
To enter our contest, you must either register your MySmile account or visit your dentist’s office for a teeth cleaning between June 14 and November 13 2019.
For each game, we will randomly select one winner. At that point, their entry into the contest will be removed and those who enter will have a chance to win during each drawing until all the tickets have been given away.

Official Contest Rules, Terms and Conditions
ELIGIBILITY No purchase is necessary to participate in the Sweepstakes, which is open only to legal residents of Washington state who are 18 years or older at time of entry, who have dental benefits with Delta Dental of Washington (“DDWA”) and have a registered MySmile® account. To learn about the benefits of a MySmile account and to register, visit our website at https://www.deltadentalwa.com/tools-and-resources/online-account-benefits. Purchasers of dental benefits through www.deltadentalcoversme.com are not eligible for this promotion. Employees, officers, and directors of DDWA and its affiliates, the NFL Entities (as defined below), Seattle Sounders FC, Major League Soccer, and all other entities directly associated with this Sweepstakes, and the immediate families (spouses, parents, children and siblings, and their respective spouses, regardless of where they live) and individuals living in the households of the above, whether related or not, are not eligible to enter or win. Void where prohibited by law. The Sweepstakes is subject to all applicable federal, state , and local laws and regulations.
HOW TO ENTER To enter the Sweepstakes, you must either: (i) register for the first time for a MySmile account, or (ii) if you are already have a MySmile account, visit your dentist’s office for a preventive care service from your dental provider between June 14 and November 13, 2019. You are eligible to win the prize tickets only for the period you meet the eligibility requirements. For example, if you registered for MySmile for the first time on July 18, 2019, you were not eligible to win the July 17 Sounders FC tickets listed below.
PRIZES The Sweepstakes will begin June 14 and conclude November 14 at 5 p.m. Pacific time. There will be a total of four (4) prizes for this promotion consisting of tickets to a Sounders FC or Seahawks games listed below. The approximate retail value (ARV) of the Sounders FC tickets is $320, and the ARV of the Seahawks tickets is $1500. The prize is non-transferable, and no exchanges are available. Odds of winning will vary by the number of eligible entries received. ONE ENTRY PER MYSMILE ACCOUNT. If more than one (1) entry is received per account, only the first entry received will be entered into the Sweepstakes. All other entries received over the limit will not be accepted. Winners will be notified via the email address associated with their MySmile accounts.
GENERAL CONDITIONS: Winners and their guests must be able to attend the game represented on their winning tickets, on the dates stated above, or the prize will be forfeited and awarded to an alternate winner. Sole responsibility for transportation and parking at game is that of the winners and their guests. If for any reason whatsoever any portion/tickets of the Prize are not used, no compensation or substitution will be provided for the difference. No cash redemption, substitution, or transfer of prize permitted. All federal, state, and local taxes on prize and expenses relating to acceptance and use of prize are the sole responsibility of winners. Prize winners will be selected via a random drawing conducted at Random.org using a list of all eligible entrants. Prize winners will be notified by email at the address associated with their MySmile account. If a winner has not acknowledged receipt of the email after two attempts, the prize will be forfeited and may be awarded to an alternate winner. By participating, entrants agree to: (a) be bound by these official rules and (b) release and forever discharge and covenant not to sue Delta Dental of Washington and their affiliates, subsidiaries, distributors, agents, advertising & promotion agencies, and the employees, directors and officers of each of the foregoing entities from any and all claims, demands, damages, losses and liabilities, of any nature whatsoever, which they may now or hereafter assert, including but not limited to claims for injury, death, damages, or illness arising out of or relating to or claimed to be arising out of or relating to promotion entry, prize acceptance, or use or misuse of the prize or any product or prize certificate awarded including, without limitation, personal injury, death, and property damage. Acceptance of a prize constitutes winner’s agreement and permission to the use of his/her name and/or photograph and/or submitted content for advertising, publicity, and promotional purposes without further notice or compensation (unless prohibited by law).
