Online proposal: To use this tool, please complete and return the Online Proposal Application prior to registering. You will not be able to register online until application is completed. Contact Producer Licensing at (206) 528-2323 or email ProducerLicensing@DeltaDentalWA.com for assistance.
Online reporting: To use this tool, please complete and return the Online Reporting Application & Change Form. Available only to groups with 100+ employees. Contact us for assistance at (800) 403-6101 or email Webinfo@DeltaDentalWa.com.
Online enrollment: Delta Dental offers 2 types of access to our Online Enrollment tool:
- For groups electing to exclusively manage their eligibility information via our Online Enrollment tool, rather than using paper enrollment forms or electronic file submission, please complete the Online Enrollment Application & Change Form and fax it back to Delta Dental at (206) 985-4783.
- For groups that submit their eligibility to Delta Dental via electronic file or paper enrollment forms, Delta Dental offers View Only access to our Online Enrollment tool. To sign up, please complete the Online Enrollment Application & Change Form and fax it back to Delta Dental at (206) 985-4783.
