Questions on how to become an Online Enrollment Group or about the Online Enrollment tool? We offer 2 types of access to our Online Enrollment tool:
- For groups electing to exclusively manage their eligibility information via our Online Enrollment tool, rather than using paper enrollment forms or electronic file submission, please complete the Online Enrollment Application & Change Form and fax it back to us at (206) 985-4783.
- For groups that submit their eligibility to Delta Dental via electronic file or paper enrollment forms, we offer View Only access to our Online Enrollment tool. To sign up, please complete the Online Enrollment Application & Change Form and fax it back to us at (206) 985-4783.
