Benefit Plan Summaries
Your Delta Dental PPO plan gives you access to two NATIONWIDE dentist networks - Delta Dental PPO and Delta Dental Premier®. You're welcome to use any licensed dentist you want, but, if you select a dentist who is part of one of these networks, your out-of-pocket expenses may be lower.
Delta Dental PPO Dentists
Dentists who participate in the Delta Dental PPO network have agreed to provide care based on a lower average fee schedule. You cannot be charged more than these fees. PPO dentists complete claim forms and submit them directly to us. You are responsible only for your deductible, coinsurance and/or amounts in excess of the plan's annual maximum.
Delta Dental Premier Dentists
Don't worry if your dentist is not part of the Delta Dental PPO plan. Chances are, he or she is part of the Delta Dental Premier network. Delta Dental Premier dentists also have fee arrangements with Delta Dental and agree not to charge you more than these pre-approved fees. They will submit claims for you, and we'll pay them directly. Again, you are responsible only for your deductible, coinsurance and/or amounts in excess of the plan's annual maximum.
