Member dentists can file their fees one of two ways, either taking advantage of our new on-line fee filing application by clicking on the File Fees tab at the top of the screen, or by completing a paper fee survey.

Member dentists choosing to use a paper fee survey should call Provider Services at (800) 238-3439 to request a survey.
