Thank you for considering Washington Dental Service. To receive a WDS quote for your client, you first must be appointed by Washington Dental Service with the Washington state Office of the Insurance Commissioner (OIC).
To apply for an appointment, you are required to complete the following forms, which can be downloaded:
You can also email us at Producerlicensing@deltadentalwa.com or call (206) 528-7351. Please return the completed forms to us along with a copy of your current Washington state agent/broker license.
Please either fax the forms to (206) 985-4821 or email them to producerlicensing@deltadentalwa.com.
Upon receipt of all the necessary forms, Washington Dental Service will initiate steps with the OIC for your appointment. At the same time, we will send you a welcome kit that includes detailed information on all of our products. Please allow at least 10 business days for the OIC's Department of Licensing to return the appointment certificate. Washington Dental Service will forward a copy of the appointment certificate to you upon your request.
Please remember that, once appointed, you are required to present your Washington Dental Service agent appointment certificate to clients upon their request. The OIC publishes licensing information for agents and brokers on its Web site at www.insurance.wa.gov.
