PLAN DETAILS

• Voluntary Plans have a 12 month waiting period for Class III and orthodontia if a group has not had prior coverage or for employees/dependents coming on to the plan after the initial effective date of coverage.
• The PPO Basic Plan has a 12 month waiting period for Class III for groups with 5 to 9 enrolled lives.
• The Voluntary Plan deductible applies to all classes.
• Voluntary Standard Plans include periodontal and endodontics in Class III and Voluntary Enhanced Plans include those services under Class II.
• Ortho available to groups with 10 or more enrolled employees.
